New Hires

New hires are eligible for insurance beginning the first of the month following 30 days of employment.  As a new hire, you are eligible for guaranteed issue Supplemental Life Insurance (to a maximum of $100,000), Voluntary Policies, including Short Term Disability and Cancer Insurance, and Long Term Disability at the Gold Level.  In the event that you change these further down the road, you will be required to fill out an EOI (Evidence of Insurability).

Status Change or Other Qualifying Life Event

Status Changes include changes from a non-benefitted position to a benefit-eligible position.  In the event that you have a qualifying status change, you will be given 30 days to complete your enrollment.

Other qualifying life events include the birth of a child, adoption of a child, marriage, divorce, a dependent has gained other coverage, a dependent has lost other coverage.  If you have one of these qualifying events, please contact Personnel.  With the necessary documentation, we will ensure your benefits are updated appropriately.  Please keep in mind that you have 30 days to make changes to your pre-tax insurance elections following a qualifying life event.

It’s important to note...

When your benefit amount for life insurance rises over $100,000, you are required to fill out an Evidence of Insurability (EOI).  Also, if you are enrolling in the Long Term Disability Gold Plan, or any other voluntary policy and you are not a new hire, you are required to fill out an Evidence of Insurability.  Please obtain the Evidence of Insurability (EOI) form in the Forms & Documents section of the SGMC Benefits Portal and submit to Cigna.

**When you have completed registration, click on MyADP, or when you receive your confirmation email, click on MyADP to access your Benefits Enrollment.

Allstate Benefits can be accessed by going to https://awd.benselect.com/enroll.  Use your Employee ID and your PIN to log in.  Your PIN is the last 4 digits of your SSN + the last two digits of your birth year.  If you need assistance with your Allstate Benefits Enrollment, please refer to the Allstate Benefits Enrollment Assistance Guide in the Quick Links Section of this page, or reach out to a Taylor Insurance Representative.  Contact Information is below.
