You can complete your Open Enrollment one of two ways: Online Self-Service or One-on-One.


If you would like to complete your Open Enrollment online, please follow the link below and log in with your Employee ID Number and PIN (all PIN numbers will be updated Saturday 10/11/14 to reflect the last four of your SSN + the last two digits of your birth year):

If you would like to complete your Open Enrollment One-on-One with a representative of Taylor Insurance Services, please follow the link below to make an appointment.

Start by choosing your department, and then a location for your appointment.  This will bring up a calendar for you to choose a date for your appointment.  The days in white are available for that location and time you selected.  The grayed out days are not available for that time and location.  Select your appointment time by clicking “Book It.”  Your Department Manager has also been emailed a copy of the schedule that includes all locations as well.


We understand that you are very busy and may need a friendly reminder for your appointment time, therefore, it is strongly recommended to use the Text Message Reminder available through the Appointment Plus scheduling system!



PLEASE NOTE THAT YOUR OPEN ENROLLMENT MUST BE COMPLETED BY FRIDAY, NOVEMBER 14, 2014. IT IS IMPORTANT TO REVIEW ALL BENEFIT PLAN ELECTIONS AND BENEFICIARY DESIGNATIONS FOR ALL PLANS YOU WISH TO CARRY FORWARD INTO THE 2015 PLAN YEAR.



DUE TO AFFORDABLE CARE ACT (ACA) REGULATIONS, SGMC IS REQUIRED TO HAVE UPDATED DEPENDENT SOCIAL SECURITY NUMBERS IN OUR ENROLLMENT SYSTEM.  YOU MUST PROVIDE THIS INFORMATION DURING YOUR ENROLLMENT FOR ANY AND ALL DEPENDENTS YOU ARE ENROLLING IN THE MEDICAL PLAN.  IF THIS INFORMATION IS NOT PROVIDED BY THE END OF OPEN ENROLLMENT ON NOVEMBER 14, 2014, COVERAGE FOR DEPENDENTS WILL NOT BE IN FORCE EFFECTIVE JANUARY 1, 2015.
