HEALTH INSURANCE

CHRA & BIOMETRIC SCREENINGS 2014


During each Open Enrollment, benefit-eligible employees are given a choice of three Health Plans with different provisions and costs:


Plan 1 - Basic Preferred Provider Organization Plan (PPO)

Plan 2 - Premium PPO

Plan 3 - Health Savings Plan (HSA)

To be eligible for enrollment in Plans 2 or 3, the completion of an annual confidential Clinical Health Risk Assessment (CHRA) and biometric screening is required of all participating employees and their spouses. 


The CHRA will give you a personal snapshot of your current health status.  Eligible Plan Members who choose not to complete the CHRA and/or biometric screening will still have the option of enrolling in Plan 1.  Remember, your spouse must also complete the CHRA and biometric screening to be eligible for coverage under SGMC Plans 2 or 3.
